  • Management and implementation of a Physician ordered Plan of Care

  • Conducting the Initial Admission Evaluation and Care Plan visit

  • Regularly Evaluating the Client's Nursing Needs 

  •  Interaction with the client's family/caregiver regarding client care

  • Evaluation of the client and communicating changes in client's condition to their physician

  • Furnishing those services requiring substantial and specialized nursing skill

  • Initiating appropriate preventative and rehabilitative nursing procedures

  • Educating our clients, family members and/or primary caregivers on how to manage treatment regimens

  • Supervision of Nurse Aides

  • Personal Care (Bathing; Dressing; Grooming; Toileting; Continence Care)

  • Medication Reminders

  • General Light Housekeeping

  • Errands

  • Accompanying Client to Appointments

  • Reading and Recording of vital signs such as: Blood Pressure, Temperature, Pulse and Respiration

  • Assisting our client in meeting personal hygiene needs

  • Assistance with bathing

  • Assistance with dressing

  • Preparing meals and assisting with eating (if necessary)

  • Assistance with toileting

  • Assistance with oral care

  • Medications Reminders (self-administered)

  • Assisting client with mobility and skilled transfers using proper body mechanics
